Effective January 1, 2020, any individual student traveling on Temple Affiliated Travel will be required to obtain approval from Risk Management and Education Abroad prior to departure.
Any individual professional, graduate, or undergraduate student may request approval to travel internationally for the 2021-2022 academic year. All requests are submitted online through this form. Requests are done on a rolling basis, and should be submitted no later than 30 days prior to travel dates. Late submissions may not be accommodated.
Submissions for approval will need to include the following:
- COVID-19 Individual Health Preparedness
- Description of Travel, Itinerary & Activities
- Academic or Research Continuity
- Department of State, CDC and International SOS ratings of proposed destination
- Approval from Division to Travel
